Care to support independence and wellbeing.
We have a range of services to provide you with support in your home and in the community, whether you are a carer, receiving care, or looking for some additional help to manage day to day activities.
We can help provide practical support, access to information or other support services as well as peer support and social opportunities.
Our support services include:
- Home and Community Care Program - tailored care to support those under age 65* to remain independent
- Support for Carers - provides a range of services to support carers to manage their caring role and improve their wellbeing
- Carer Gateway - a national initiative program that offers a diverse range of short-term services to help you manage your caring role
- Foster Care - every child deserves a safe and supportive home where they can thrive
- Disability Social Support in Wimmera - day programs, one-on-one support and short term accommodation to help you build the life you want.
Support to feel your best
Whether you receive support or care for someone else, we can help you make the most of your current situation.
Knowing you’re not alone is important to maintaining your overall wellbeing. Everyone’s situation is different.
If you are a carer this may include caring for:
- a child with a disability
- a sibling with mental health needs
- a relative with a chronic health condition
- young people caring for a parent or sibling with a health or social issue
- a neighbour who needs extra support
- a loved one with mental health support needs
- a member of your community experiencing alcohol and other drug use dependence.
If you require support, you might need assistance in your home to live independently due to:
- physical disabilities or chronic conditions
- mental health challenges
- feelings of social isolation.
We can help with short-term or long-term support depending on your individual situation.
