The Home and Community Care Program helps you access the support you need to optimise your health and wellbeing, while maintaining or regaining your independence and remain active in the community.
Home and Community Care can provide support for you to remain independent. Services are integrated so care can be tailored to meet your needs and changing circumstances.
What we can support you with
We can help you with daily chores, occasional jobs around the home, or making daily life easier and safer. This may include maintenance related to safety security and access, minor works – help with managing mobility issues to move safely around the house, such as installing grab rails or checking smoke alarms.
We’re also here to help you keep doing the things you enjoy and stay in touch with others with planned activities, one on one support, home visits, personal care (where available) and information or referrals to other services.
Supports offered include:
- Domestic assistance: Daily chores and occasional jobs around the home
- Cooking program: Help to prepare and store meals
- Simplify and organise: Help to declutter and tidy your belongings
- Managing mobility and safety: Safety and security around the home, such as installing grab rails or checking smoke alarms
- Social activities: Group activities to keep you connected to your community
- Volunteer support and home visits: Companionship and assistance when you need it most
- Respite: Help with respite to care for a loved one
- Personal care: Personal care and hygiene support
- Living at Home Assessment: A holistic assessment to understand your needs before developing a care plan
- Information or referrals to other services: Learn about the supports provided by allied health and other community services providers.

Who can access Home and Community Care?
After a personalised assessment, we can help you and your family or carer identify your needs and choose the right mix of services to help you live as independently as possible. Regular reviews ensure the services that you receive are relevant and meet your changing needs.
To be eligible for this service you must be:
- Under age 65 (or under 50 for Aboriginal and Torres Strait Islanders)
- And at risk of losing your independence due to disability, chronic medical condition or mental health challenge.
Home and Community Care is a needs-based program and eligibility does not guarantee access. Services vary from region to region.

Home and Community Care assessments
Regular assessments can help meet your changing needs.
We can help you and your family or carer identify your needs and choose the right mix of services to help you live as independently as possible. You will receive an individual care plan, with scheduled reviews so that your care plan changes with your changing needs.
A Living at Home Assessment is an in-depth holistic view into your daily life, with the goal to explore ways to live independently and remain active in your community.
The Living at Home Assessment establishes your eligibility to receive Home and Community Care funded services and supports referrals as appropriate.
If you have more complex support needs, you can access case management through linkages.
Linkages can help you access services to develop your skills and build your capacity towards independent living. Linkages also support you if you are transitioning to another funded service under National Disability Insurance Scheme or My Aged Care.
